Hello Parents and Student-Athletes,

We need your help in completing and submitting the Athletics Pre-Participation Paperwork, physical exam and activity clearance for students with medical conditions and/or implanted medical devices.  We are excited to now have the Athletics Pre-Participation Paperwork available ONLINE!  If your child will participate in Athletics during the 2020-2021 school year, you must complete and submit the online form as well as print the attached medical history, physical exam, & activity clearance for students with medical conditions and/or implanted medical devices forms (PDF) and take with you to your child's doctor appointment.  Please note that the Student Health Center does not provide sports physicals.  All forms must be completed before participating in tryouts, practices, or contests.  

If your child is not sure about participating, please go ahead and fill everything out now to avoid confusion later in the case they decide to participate later in the year.

All forms must be submitted and returned to the Athletic Trainer prior to participation in athletics, ideally return by August 1st to the TSD Athletics Department to ensure that we have received your child(s) paperwork before the first practice of the season. As a matter of school policy, all students participating in athletics must have a new physical on file each year of participation. Please submit the physical by August 1st to Robert Cadotte, Athletic Trainer.


Students in grades 6th-12th are required to be registered the Rank One Sport software which will be mandated TAPPS wide starting with this upcoming school year.

Students in grades 3rd-5th aren’t supported by Rank One Sport and thus their athletic clearance will be maintained through excel, like years prior.
